- The URL of the facility or guide page.
- The name of the facility and the city and state, if relevant.
- Exactly what appears incorrect.
- A link to an official hospital or government page supporting the correction, if available.
Please do not send symptoms, medical records, insurance IDs, Social Security numbers, payment card information, or other sensitive personal information. Your message is emailed to the site administrator for a website-related response; it is not an emergency service.
